ICD-10: L20 Atopic Dermatitis vs. Data Field 131720: Date L20 first reported

I am studying atopic dermatitis and I found a discrepancy between the numbers reported for the date L20 (ICD-10 code for atopic dermatitis) first reported and the number of subjects with ICD-10 L20 code.

 

For Data Field 131720: Date L20 first reported, there are 13,926 individuals who have an input for this field.

 

L20 is the ICD-10 code for atopic dermatitis. So then, I went to the ICD-10 code field to look at L20 to see how many people have L20 reported in their records and its only 272 people.  

 

I am wondering why there is a discrepancy between 13,926 individuals and 272 individuals . I tried looking into the notes in the data fields in the UK Biobank showcase to see if there was some explanation but I couldn't find anything explaining the discrepancy. I was thinking maybe the Data Field 131720 also includes people who have self-reported atopic dermatitis, but that is not the case as I have also checked the source of report of atopic dermatitis Data Field 131721 and it says they were all reported from primary care only. 

 

Only 272 out of 500,000 people would be really low for prevalence of atopic dermatitis, hence I am using the Data Field 131721 for identifying subjects with atopic dermatitis.

 

I wanted to see if you have any clarification on this.

    Data field 41270 lists the ICD10 diagnoses that were made in hospital. The Notes tab on the Showcase page for field 41270 has an explanation of this. There are 272 participants with L20 in this field on the RAP.

    For most participants, their atopic dermatitis is diagnosed by a GP (primary care). As you point out, the primary care diagnoses are listed in field 131720. For more detail of the primary care records, see also the record-level data in field 42040, but note that the diagnoses in field 42040 are listed with Read codes not with ICD codes.
