How should an applicant apply if collaborator is affiliated with multiple institutions?

if a collaborator is affiliated with multiple institutions but could only listed one institution during the application, is it possible to include all the institutions when publishing the article?

    Dear Xin Ji, 

    Thanks for your post on the UK Biobank Community Forum. 

    On the application you must list all institutes where you will be accessing UK Biobank data. If you are affiliated with multiple institutes, it is possible to be added as a collaborator on the application at multiple institutes as long as you have added those institutes to your registration.

    Please note, if you add a collaborating institute to your application we will require the institute to agree to a Material Transfer Agreement.
